Alpha Care Health Services of Wisconsin provides a qualified and licensed Medical Social Worker services. These services shall include but are not limited to:

• First visit within one week of referral by a physician to determine immediate care and support needs of you or your loved one
• Thorough initital assessment, reassessment of you or your loved one
• Referral to community resources
